This song is actually about Amy Lee's sister, Bonnie, About how she died.

"How can I pretend that I don't see
What you hide so carelessly?
I saw her bleed"

She is saying that she knows that she is dead even if you try to hide it I will always know I saw her die.

"You heard me breathe and
I froze inside myself and turned away
I must be dreaming"

She says that when she saw what was happening she hoped it was a lie.

"We all live
We all die
That does not begin to justify you"

That means that even if everyone dies doesn't mean that it was right for her to have died even if "we all die"

I have heard this song over and over again and I think I'm going to have to disagree with you on that one.I believe this song is about Amy (or a character) dating a guy and she saw him kill a girl and she's trying to understand what's going on because he keeps telling her she was dreaming but that's not working on her. So she knows how he has been brainwashing her and blaming her "visions" on her mentality when it was actually true. So I believe the ending is slow and eerie because I believe he kills her.

I think this song is about Amy being with a "perfect" boyfriend. At one point, she say him abuse or murder another woman, and she's in shock. She is telling him that even though no one else knows, other people have done the same thing, and she loves him, she doesn't want to let him get away with it. She's trying to convince herself that this guy could never do something like this. It wasn't him. I saw you do it, and you know I saw you do it, but you didn't do it, so it's okay, right?

Actually I think it may be about someone (it does not have to be Amy herself) that is in a relationship.
How can I pretend that I don't see
What you hide so carelessly?
I saw her bleed
You heard me breathe
And I froze inside myself and turned away
I must be dreaming
- She probably saw her love do something brutal to another woman (My guess is murder, thus the 'bleed') And she did not want to belive that this is true. Her partner noticed that she saw this ('You heard me breathe) She got scared, ran away, and now she's still telling herself that this is a dream.

We all live (and)
We all die (but)
That does not begin to justify you.
-That is the character telling her partner that what he did was wrong, while he may have said -It's nothing. She was going to die anyway- and she told him that it 'does not begin to justify you'

It's not what it seems
Not what you think
No, I must be dreaming
It's only in my mind
Not real life
no, I must be dreaming
-The girl is trying to tell herself that this isn't true. She doesn't want to believe it. She's screaming (I actually think that the high voice of Amy here symbolises the shrieking of the girl) 'No! I must be dreaming!'

Help I know I've got to tell someone
Tell them what I know you've done
I fear you
But spoken fears can come true.
- The girl is terrified. Her partner tells her not to give him out, but she knows that she needs to. The 'I fear you' might mean that she knows what he is able to do, and is now afraid of what he'll do to her if she gives him out. 'Help' because she is scared, of her partner and of what he did. 'Spoken fears can come true.' She fears that if she doesn't give him out herself, it may turn against her/ or it will get even worse.
