Upon first glance Fixxxer appears to roam in a Metaphoric state about drug abuse, as known, the lyrics of Load/Reload is soaked by Hetfields personality and if you know his choosing of lifestyle you should know he is highly addicted to the power of drugs. The pins may symbolize a needle or a tool of torture inflicted by "The Fixxer".. A Fixxxer may be a person who pushes the individual to the point so that he/she wants to escape from "what fathers done". There is more to it but I accually have a life and a breakfast chopped on a mirror, I'm ready to ride the tiger and I know the stripes are clean... Of to the stairway to heaven to meet my Fixxxer...

I think fixxxer is about james childhood and how his dad left him when he was young, and about his mom dying of cancer and how he is lost without them in his life.
in the song he says can you fix what fathers done or fix this whole in a mothers son he's asking for help a release from his pain and loneliness.
