This song is talking about rebelling against authority. The author wants to "scream louder and louder and louder" against the stiffling and conforming atmosphere of society.

The phrase "Hammer to Fall" is referring to that one day, people will not tolerate authority's stiffling power any longer, and they will rise up and overthrow authority. Hence, the hammer falls.

Brian May said himself that this song is about the end of the world. My interpretation of how the song says the world will end is by war:

"Till one day they call your name" (draft)
"In the shadow of the mushroom cloud"
"What the hell we fighting for ?"
