It's a breakup song. "Girlfriend in a coma" signifies that he has given up on their relationship, she's practically "dead" to him and there's nothing else that he can do make it work.

I think it's pretty obvious that the guy is one of those who regularly beat up their girlfriends. Only this time he's gone a bit too far and now she's ended up lying in a coma.
He has a problem admitting his responsibility. That's why, though conceding that "there were times when [he] could have murdered her", nevertheless he would "hate anything to happen to her".

It's a pretty literal interp. The guy's girlfriend is in a coma and knows she's going to die, yet he is in denial and going through some guilt. He probably physically hurt her but didn't want to get caught. Hence "there were times when I could have murdered her, but I don't want a thing to happen to her." Out of guilt he refuses to see her. At the end he comes to terms that "it's realy serious" and she is dying so he begs to see her for the last time.

YES!! that's it! and at times he wanted to murder her, other times he could have strangled her, but in reality, he would hate anything to happen to her. That sums it up
