It's a pretty literal interp. The guy's girlfriend is in a coma and knows she's going to die, yet he is in denial and going through some guilt. He probably physically hurt her but didn't want to get caught. Hence "there were times when I could have murdered her, but I don't want a thing to happen to her." Out of guilt he refuses to see her. At the end he comes to terms that "it's realy serious" and she is dying so he begs to see her for the last time.

I thought his girlfriend was in a comma because she fell out of a window while reading that book "Eats Shoots and Leaves" ... no wait. That can't be it.

I think it's pretty obvious that the guy is one of those who regularly beat up their girlfriends. Only this time he's gone a bit too far and now she's ended up lying in a coma.

He has a problem admitting his responsibility. That's why, though conceding that "there were times when [he] could have murdered her", nevertheless he would "hate anything to happen to her".
