This is just my interpretation, if you have anything you think should be different, let me know.

From the very beginng we know this song is aimed towards the devoted fans that never tunred their backs on Fall Out Boy, becuase of the words Jay-Z is saying at the beginning of the song.

"Last summer we took threes across the board"
Last year Fall Out Boy had three major hits on the bill board charts, those being A Little Less Sixteen Candles, Sugar We're Going Down, and Dance Dance, though all of the songs on FUCT were amazing. The board, is referring to the bill board charts."But by fall, we were a cover story now in stores" That fall, they made a name for themselves like never before, and everyone knew the name of their album."Make us poster boys for your scene, but we are not making an acceptance speech."Fall Out Boy has basically been the poster band for Emo/Punk Rock music, and they are saying that yes, you can make them the poster band, but that there not doing this for the fame. They are doing this for the music.

The chorus goes "So long live the car crash hearts, cry on the couch all the poets come alive, fix me in 45" This is basically saying "long live the broken hearts, who cry all night and put in this cd, to fix you." "Fix me in 45" does not just seem to be a random number. All three of the major Fall Out Boy albums have been approximately 45 minutes long."I can take your problems away with a nod and a wave of my hand, 'cause that's just the kind of boy that I am" This is referring to all the fans who idolize them, and think of it as an escape from all of their problems. "Crowds are won and lost and won again but our hearts beat for the diehards" Here they are saying that some people love them, some people hate them, but they play for the music, and what it does for the fans.

I agree with a lot of the interps so far... I just have one thought... That differs... everyone is saying 45 minute therapy... 45 minute album length .45 caliber gun.... okay those are all valid... but I always thought of the band as really being about the music and having quippy double meanings so it may mean all of those things but what I thought about when I first heard the song was an actual LP record... 45... like a way to connect themselves with music immortality.

The 45 rpm single records are made from vinyl plastic that is flexible and unbreakable in normal use.

Ok so you guys keep getting parts of it wrong, I just got the CD yesterday(xmas) and it has lyrics in it ....it says

i found the safest place to cheat all of our mistakes
every DOT COMs refreshing for a journil update

i take it as like some1 at some sort of therapist becuase a therapist has ou sit on a couch and you get like 45 mins

the beggining is like he is tellin his story....
later it says

I can take your problems away with a nod and a wave
of my hand because that's just the kind of boy that I am
the only thing I havnt done yetis die
and its me and my press my press one at the afterlife
...meaning its the therapist and he is sayin he's taking away the stress and how he is tired of his job and he calls in his next person to hear something new

This song I believe is about becoming famous and not wanting all the things in between such as selling out to make more more. They don't want to be poster boys and have false lives they want to continue to do music because they're passionate about it not wanting to make money for it.

I rather be poor and love what I do than rich and become resentful.
