I think this song is about people moving along with the crowd and are blinded by the fact that they don't really understand whats going on around them.its like saying that when the people around you really want you to understand them you dont, just like Cobains lyrics say,"Knows not what it means when I say AHHH"

It's about his feelings about his best friend Dlyan Carson. Despite their frendship, Kurt obviously didnt have much respect for him.
