He is talking about how much his life has changed from when he was addicted" I walk like a burnt out pornstar"- referring to himself being all exhausted and miserable." With my feet begging for a car"- because he wants to move on to a better life and walking is much slower than driving of course. "My buddy had a kid with a girl named star.Amazing how the little things are."- He is comparing his current life of being stuck and feeling alone and miserable to his friend who now has a family.
He never considered how amazing and impactful the little things could be until he saw what he missed out on.Coincidental his friends' girlfriends' name was 'star' so it could also have been a nod to wishing for a better life/ good family life.Since you usually " wish upon a star" when you have a dream or desire you want to come true.
This is just my interpretation though.So i cannot gurantee if this was the excact meaning of the lyrics Justin wrote
