   They are pretty much saying that a true friend is always going to reach a hand out to help you whenever your in trouble, no matter how hard it might be.    I believe the lyrics are translating to how true friend have your back and will never let you down. I personally had this moment a couple times. Friends are like family. They will never throw you under the bus and they are with you through thick and thin

   This isn't an interpretation (I think the song meaning is pretty clear; it means always being there for and supportive of someone), but for some reason the only time they say "My shoulders are small" is the first time, and for the rest of the song they say, "My shoulders are strong."
