He knows that she has fallen for him and he looks at her in a way which makes her feel 'low' of herself.
Even with the shrude attitude he throws at her and takes her sweetness for granted, she is still in-like with him but at times she wishes she was the other girl who is unaware of these happenings, whom 'He' is cool with..

I think this song is about a relationship with an unfaithful partner. In the song, Adele describes being in a relationship with a guy who she knows is unsatisfied with the relationship hence the reason he's cheating.
'Cold Shoulder' is about a boy who has been holding back, maybe because of something she had done in the past; but he's refusing to admit it, as 'You say it's all in my head, that the things I think just don't make sense' states. Shes noticing that he's been ignoring her and starting to like someone else, 'You grace me with your cold shoulder, whenever you look at me and wish I was her."
In the second verse, it states, "Do tell me why you waste our time, when your heart ain't in it; and your not satisfied;" Saying that he's confused right now too- He knows he doesn't like her, but he still isn't satisfied for whatever reason.
