The meaning isn't gonna be very complex/complicated since this was probably written on a parking lot the day before it was recorded. Also, I think I read somewhere that "Scoff" is a "dishing out to [Kurt's] parents.
So I think it's about a neglectful father who's never satisfied with his kid. My father is just like that, so when I heard this, I automatically thought "Yeah... yeah... that's what it feelsd like. Exactly like that. Wow, the emotion feels so authentic here. So simple and yet to the point."

"In my eyes, I'm not lazy" -> Very simple, a father complaining about his son being oh so lazy/never being satisfied with him while the son sees it differently/ does not feel his father noticed/recognized his effort/how he gave his best (For example a dad telling his starving-artist of a kid to get a proper job, and the kid considers Art to be a proper job already)

"In my face, it's not over" -> The kid is forced to be quiet and do as the father says, but is still angry/doesn't consider the argument over. Or its the father who further enrages the kid by saying something like "Don't think this is over yet, young man"...

"In your room, I'm not older" -> When the father summons the kid to speak with him/scold him, he makes his son feel like he's a helpless little kid again ("...not older...") - or it's that the father keeps treating his son like a little kid.

"In your eyes, I'm not worth it." -> pretty straightforward. All too much use of the phrase "Son, I am disapointed".

"Gimme back my alcohol" -> also pretty straighforward. The father found his son with something that definitely doesn't belong in the hands of teenaged kids and confiscated it, his son wants his booze back i.e. considers that the father has lost the right to act like a parent and forbid him things when he doesn't really care about his son at all/ thinks that the father has no right to complain about his son turning out wrong since it's his fault for neglecting him.

"Kill a million, heal a million" - > Bad parents who don't realize they are always tell you how much they did for you/how much they worked to fill your ingrateful mouth. But that doesn't "count" if they did about the same amount of damage to you.

i think this song means, like most of nirvanas songs, 'it's however you perceive something' the lyrics are the perfect example 'in my eyes...in your eyes'. then he goes to say 'don't control me and let me do my own shit if i wanna drink i can, dont tell me its bad, good, whatever it's in my eyes' also the 'kill-heal' part means heres the proof whatever i do has a positive and negative reaction its just whatever your stance is on it. so this song is actually kinda about physics too. and just the general meaning of life. & thats why we love kurt because his songs are about such offbeat things.
