    He (the narrator) could've witnessed/ known someone who tried/succeeded in taking their life on the rails. After he moved to London he could've gotten asthma from the pollution. This song could also be informing the listeners about what goes on in London. On Genus Lyrics an interview was done with Wilbur Soot and he said that he had gotten asthma after moving to London.
