This song is about a guy who is in a relationship with this girl, but he really feels like its not working out because "even though you're next to me I still feel so alone... I just can't give you anything for you to call your own". In otherwords, they have this relationship but he just can't seem to invest the emotion in it that he knows he should, and he feels like he's letting this girl down because he doesn't feel the same way about her that she feels about him.
So he can hear her breathing as she lies next to him and it's keeping him awake because of the guilt/anger/confusion/emotional turmoil he's feeling in this relationship.
