The refrain is a reference to physical violence in relationships. Cooper is saying that only women bleed through the act of loving the opposite sex.

Women can get pregnant, they have a womb, which references the vulnerabiity lyrics, and the bleeding. Over many years of patriarchy if a woman does give herself to a man, she does not find it easy to leave even if abused, due to the children. this is still relevant for nowadays, because even with contraception giving women some more free choice, it is still mainly female children who endure child abuse and this often infringes on the freedome of all girl children. It can be hard for some women to open up to be the warm lover they wish to be, after years of being told to stay indoors as a child, or of possibly having to overcome situations like date rape or some abusive episode from their childhood. When they need a man to be patient to help them reach potential sometimes they end up with a man who is egotistical and angry if they are not his ideal hot woman straight off. The woman thus by nature of her being touched inside during sex is more involved sometimes in the connection, more fragile about having that connection. Some women get ripped apart in childbirth and never feel the same, or nerve endings are damaged in cesarian, or they have fibroids or adhesions. For those women sexuality is simply bleeding and not ever much fun at all. Women can suffer when they commit to a guy. A guy who cares treats that woman with kindness and flowers to respect that she needs some sweetness from him.

This song is not about a woman's period, however, using the phrase "only women bleed" helps develop the idea that women are fragile. The metaphor could even extend to include a girl losing her virginity. These connotations reinforce the main idea of the song (domestic abuse) and evokes empathy from the listener by showing the pain and suffering that a woman faces in life.
