He's been having a really bad life, and he's telling us that he's planning on changing his life around. But he has to start by getting help. "Waiting on a second-hand pick-me-up" refers to him using drugs, and he's trying to get help so he can continue on to make himself better. "I've been going crazy, I don't wanna waste another minute here" he realizes what he's doing is wrong and he is leaving wherever he is to change himself for the better.
