I believe that this song is about the delicate mind of a depressed person. How if you pull to hard on it (add to much stress) it will snap. Or how if you leave it slack (leave themselves to their depressed thoughts) then it just won't hold. Like how the mind of this person is like their own private hell. They scream out for help yet, feel that know one will listen. They search for anything, anything to keep them from getting more depressed and they find it in their hearts, their fear drenched hearts. And how if you fill their glass to the brim (give them more than they handle) they'll spill it out. And if you keep sharpening the knife (pushing them to the limit) they will fall back and be "so blunt". How they have to learn to live with their lives or leave it. And the last part, if you try to hard to understand what they are going through, you will never know
