I think this song is about how people can handle situations. How anyone has the power to hurt something or someone, and they have the power to help. It's up to them, their hammer is their choice. They have that power.

I feel like this song about someone attempting suicide and being there before it happens and attempting to stop them

I feel like Alec is expressing the “Hammer” as his friend. Friends can do everything a hammer can. Maybe Alec is asking his friend(hammer) what will he do next? What is his plan for this friendship? Will he break it or keep it together(create). But it’s just my opinion.
